Congratulations on the birth
of your new Little Texas!

In the first couple of days after the birth, while you are tucked into bed learning to care for your newborn and yourself, we’ll be working on paperwork behind the scenes. One of the things we do is prepare your baby’s birth records and send them to Texas Vital Statistics so they can create the legal birth certificate. This link tells you more about what’s involved in that process.

Step 1: Fill out the “Birth Certificate” tab on the portal. Put as many details on there as possible.

Step 2: After the birth we will enter the information on that form into the Vital Statistics Database. We will generate a draft document called “Verification of Birth Facts.” At your 1st one visit, you will carefully review it to make sure it’s totally correct. The VoBF can be used for legal identification until the birth certificate arrives.

Step 3: You’ll sign the document, we’ll Certify it and Release it to the State.

Step 4: Your child’s social security card will automatically be sent to you. It should arrive within 2-4 weeks of when we signed and released it to them.

“How do I get a copy of my baby’s birth certificate?”

You can request and purchase a certified copy of your child’s birth certificate from the local registrar’s office located in the city or county where the birth occurred, or from the Texas Vital Statistic office located in Austin, Texas. Click here to learn about both options:

A Certified Birth Certificate is a permanent legal document filed in the State of Texas that establishes your child’s identity and is used to apply for medical or government services, passports, school admission, etc.

“When will I receive my baby’s social security card?”

If you answered “Yes” to the question, “Apply for baby’s social security number?”, the birth information will be forwarded to the Social Security Administration as soon as the Texas Vital Statistic office receives the data from the hospital. The Social Security Administration then requires 2-3 weeks to process the information. A social security card will be mailed to the mother’s mailing address as provided in this worksheet. The entire process usually takes 4-6 weeks to complete.

“When will I receive my baby’s Medicaid number?”

If you provided an answer for the questions “Mother’s Medicaid Name?” and “Mother’s Medicaid Number?”, the birth information will be forwarded to the Medicaid office as soon as the Texas Vital Statistic office receives the data from the hospital. Medicaid then requires 2-3 weeks to process the information. An Infant Medicaid card will be mailed to the mother’s mailing address as provided in this worksheet. The entire process usually takes 4-6 weeks to complete.
