When you’ve decided that you’re ready to start care, please fill out this form. Our administrative assistant will reach out and create your portal, guide you through finishing the history and consents, and set up your 1st appointment!

The months and weeks leading up to birth will be packed with decisions and experiences. We have a plan to make it as simple as possible. Here are the first steps. Let’s get started!
Step 1: Share your info with us
Please fill out the Form. We’ll email you a login ID.
Step 2: Login to the Portal
Once logged in, a) fill out the CONTACT page, b) complete your HISTORY, and c) sign the CONSENTS.
Step 3: Pay the Deposit
Your deposit holds your place on our calendar. Since we limit our number of clients each month, we require a non-refundable deposit so we can safely turn away other potential clients when our calendar is “full”.
Step 4: Begin Prenatal Appointments
Our Administrative Assistant will reach out and schedule your initial prenatal appointment as soon as you have done steps 1 & 2. You can text or call her at 469.643.9432 if you haven’t heard from her within 1 business day.
